I've heard of people doing this...carefully using a bit of narcan if they felt like they were "too" high or had used too much and were on the verge of potential overdose. Obviously this wouldn't work if you just pass the fuck out and stop breathing before you have a chance to realize you did too much.
I'm wondering if anyone has done this, and what the consensus is...can you really avoid an overdose this way (perhaps with drugs that are slower building like oral meds, PST, etc)? Or are people who do this really just easing their own anxiety about overdosing, while not in any real danger?

I wouldn't think this would work for long acting opiayes as it'd biological half-life 2 min (IV), 5 min (IM) http://www.drugs.com/monograph/naloxone-hydrochloride.html
I've never used narcan (BAD HR ADVICE) but it's always smart to use a buddy system since if you od, really ok, the ambulance can't get to you fast enough.
As for "micro shots" of narcan I can't see that going anyway but poorly besides possibly saving your life (which isn't nothing). Narcan is an extremely competitive antigonish and to dose it just right to bring you back to "normal" without participating in the worst withdrawals you've ever had is a damn fine line.

I have long thought that Narcan would be perfect as a chipping aid for those wishing to flirt with opiates, hardened users and beginners alike, as a means of tolerance control.
have a shot, wait a few hours then hit the receptors with an antagonist and voila, reduced tolerance !
as long as you are not physically dependent at the time, it should fly ... make sense ?

My personal opinion is that if you're going to be fucking about with REALLY strong opies, make up an IV bag, and dilute the narcan, and keep it pinched off, with your fingers....
if you get so high you cannot keep it pinched off, the IV will start dripping, and you'll start ingesting narcan, if you stay passed out long enough it'll counteract the effects, if you wake up, pinch that shit off and you're cool. It's like a safety net for being too high.....
